Users will use the MDF form to submit MDF Requests. The form includes a set of standard fields such as:

  • Created
  • Title
  • Activity
  • Requested Amount 
  • Description 

You can add additional fields in the MDF Custom Fields settings.  


Add a custom field 

  1. Go to Settings > MDF > Request Custom Fields 
  2. Click Add Field

3. Name your MDF Field.
4. Set the Field Type.
5. Update the Description.
6. Set the Field Visibility.
7. Click OK.
8. Repeat until your Custom MDF Fields have been added.
10. Click Save.
