Channeltivity's Referral form includes the below standard fields:

You can add additional fields to the form using the instructions listed below: 

  1. Go to Settings > Referrals > Referral Custom Fields.
  2. Click the Add Field link. 

  3. Name the field and add the description you want Partners to see.
  4. Select a field type from the drop-down menu. 
  5. Add a Description (optional but recommended).
  6. Configure the Field's Visibility & Interaction settings: This section allows you to control who can see, edit, or is required to complete the custom field, as well as its availability for integrations. Each column represents a different User type: Internal, Partner & Distributor, and API & Integrations.

    For each User type, you can set one of the following interaction levels:

    Hidden - The field is not visible to the User (This setting only applies to the Partner & Distributor view; hidden fields remain visible to Internal Users by default)
    Visible - The field is visible to the User
    Editable - The User can view and modify the field
    Required - The User must complete the field before submitting the form

  7. Click OK and repeat until you have added all of your custom fields.
  8. Click Save to save your changes.
